We are experienced Geriatricians who offer a wide range of skills, expertise and services across all areas of geriatric medicine. We work in collaboration with patients, their families, general practitioners, other specialists and residential care staff to form individualised health care plans focusing on our clients’ goals of care.

Dr Elizabeth Dapiran
BMedSci (Hons), MBBS, FRACP
Melbourne Geriatrician, Co-founder and Director of Optimise Healthcare.
Dr Elizabeth Dapiran is a Consultant Geriatrician who has been working in geriatric medicine since 2008.
She currently holds appointments at Western Health and Alfred Health where she works with the Aged Care Assessment Service (ACAS), Hospital Admission Risk Program (HARP) and wound clinics.
She manages all geriatric conditions but her particular areas of expertise include wound management, the diagnosis of dementia and management of dementia related behavioural problems, and complex medical issues.
She is actively involved in wound research, registrar supervision and education in geriatric medicine.
Dr Barbara Sabangan
BSc, MBBS, FRACP
Melbourne Geriatrician, Co-founder and Director of Optimise Healthcare.
Dr Barbara Sabangan is a Consultant Geriatrician with more than 10 years experience in geriatric medicine.
She currently holds an appointment at Western Health and her broad range of experience includes acute and subacute geriatric medicine, the Transitional Care Program, the Aged Care Assessment Service (ACAS), and the Cognitive Dementia and Memory Service (CDAMS).
She manages all geriatric conditions but her particular areas of expertise include cognitive assessments, dementia and dementia related behavioural problems, complex chronic medical conditions and end of life care.
She is actively involved in teaching undergraduate medical students, and the supervision and training of junior doctors and registrars in geriatric medicine.
Dr Inese Tucker
MBBS, FRACP
Melbourne Geriatrician, Co-founder and Director of Optimise Healthcare.
Dr Inese Tucker is a Consultant Geriatrician of more than 10 years experience across a wide range of geriatric medicine settings including acute, subacute,community and subspecialty outpatients.
She currently holds an appointment at Western Health as the medical lead for the Aged Care Liaison Service, a nursing-led service providing an alternative to hospital care for residents of Aged Care Facilities. She also holds appointments as a Clinical Associate for the Severe Behaviours Response Team (part of the Dementia Behaviour Management Advisory Service DBMAS) and at Anywhere Healthcare providing telehealth consultations for clients all over Australia.
She manages all geriatric conditions but her particular areas of expertise include dementia and behaviour management in dementia, complex chronic medical conditions and end of life care. She is committed to improving access to specialist care, quality improvement through innovative models of care including telehealth.

ACCESSING OUR SERVICES
REFERRAL
A referral addressed to one of our doctors is required from your GP requesting a comprehensive geriatric assessment.
COST
For patients 65 years and older, we bulk bill the initial assessment and one follow up assessment per year. For further assessments, there may be an out of pocket expense.
For patients under 65 years there may be an out of pocket expense.
If an interpreter is required there will be an additional cost to the client.
LOCATION
We see patients living in residential aged care facilities in the western suburbs of Melbourne.
We may be able to see patients outside of this area. Please contact us to discuss this further.
